How to Fill Out This Form

Quick tip: Try to enter facts in chronological order (oldest to newest), but don't worry if you don't — they'll automatically sort by date!

Important: Each field below is for one individual fact or event. That means one thing that happened at one time. Break your story into separate happenings.

Step-by-Step:

1. Enter the date when something happened
2. Describe what happened — stick to facts, not feelings or opinions
3. Click away from the field to save it automatically
4. Then upload supporting documents for that fact (emails, letters, photos, etc.) — the upload button will appear after you save
5. Click "+ Add Another Fact" to continue with the next event

Your progress saves automatically as you go. You can close this page and come back anytime!
